The Provider Information Form

The Provider Information form is designed to gain a sense of your areas of expertise and experience with environmental organizations. 

 

The first step to becoming an ESC provider is to submit a completed Provider Information Form.  You can download it here.

 

Be sure to include any critical information that helps us to accurately match the organizational assistance needs of our groups to your skills and expertise.

Please note:

  • Our reference checks are rigorous; providing lists of previous clients who are comparable to our constituents and similar types of organizational assistance needs are helpful.  Reference checks must be completed before you become an approved provider.  References who are easily accessible and willing to comment on your skills will expedite this process. Any attachments should refer to the areas of organizational assistance and/or capacity building experience you may have. 

 

  • You will note a number of documents that need to accompany your form.  If you do not attach these documents, your Provider Information Form will be considered incomplete and our approval process will be delayed. 

 

  • ESC will notify you once your application has been approved.  Priority for reference checks is given to providers that are associated with an application.  If any agreements between a group and provider are entered into prior to this approval, ESC cannot be held liable.  Please indicate which group, if any, you are associated with in the proper space on the form.

 

  • Approved providers are encouraged to refer ESC-eligible organizations needing assistance to us.  Please tell the interested group to contact us directly and refrain from characterizing your referral as influencing our assistance decision-making process.  If you know of other consultants or trainers who are interested in being listed in the ESC resource pool, please have them contact us as well.

 

  • Once your form is approved, all of the information will be entered into our database.  We search the database by state or region to secure providers and make referrals when groups request this service.  We do not guarantee that you will receive a referral, however, we offer three different providers for each assistance request we approve.  Please note that the group makes the final decision to hire a provider.

 

  • At the conclusion of each training or assist, training participants evaluate each provider.  These evaluations are submitted to ESC where they are used to determine the quality of the assistance and the effectiveness of the individual provider.  ESC reserves the right to remove a provider from its database due to insufficient performance.  We will first review negative evaluations with the client and provider to ensure that all parties are given the opportunity for a fair exchange of information and views.
